Position Summary:

Under the supervision of the Director of Behavioral Health, the Behavioral Health Counselor is responsible for providing direct behavioral health rehabilitation services to agency’s clients.  Services to be provided include individual/group/family counseling, culturally relevant assessment, evaluation, and case management.

Duties and Responsibilities

Direct Services: 

  • Provide direct behavioral health rehabilitation services including individual/group/family counseling, culturally relevant assessment, evaluation and case management in department(s) requiring coverage. 
  • 65% of the counselor’s time will be spent providing direct services.
  • Manage a caseload of youth through older adult clients.
  • Provide services in school, home, clinic and field settings, often independently.
  • Case management services such as providing and facilitating referrals to appropriate agencies.
  • Provide outreach support for BH programs

Clinical Consultation and Training:

  • Participate in individual, weekly, clinical supervision meetings with the Behavioral Health Supervisor.
  • Participate in weekly case consultations and clinical staff trainings provided by the agency. 

Documentation: 

  • Responsible to open physical and electronic record charts with proper authorizations and consents in a timely manner.
  • Maintains timely completion of assessments, treatment plans, progress notes and outcome measure.
  • Maintains clinical records per appropriate standards and regulations.
  • Prepares other daily and periodic reports that may be required.

Other duties as assigned.

Qualifications

  • Master’s Degree in social work, psychology, behavioral sciences or related field
  • Minimum 2 years of experience
  • Ability to learn and apply clinical skills within the various Asian/Pacific Islander  and Latino cultures of the local community 
  • Knowledge of cultural issues, community resources and family dynamics
  • English proficiency and additional language fluency a plus (Vietnamese, Mandarin, or Spanish preferred).
  • Valid CA Driver’s License, vehicle for agency travel and current automobile insurance.
  • Knowledge of Juvenile Probation Department , Santa Clara County Department of Family and Children Services, Family Advocates, Social Security Agency, private insurance companies, Victim Witness and other involved agencies.
